Mechanism of Action:
Dual-pathway blood pressure regulation. Telmisartan selectively blocks the binding of Angiotensin II to the $AT_1$ receptor, preventing blood vessel constriction and lowering peripheral resistance. Hydrochlorothiazide inhibits water and sodium reabsorption in the distal renal tubules, reducing blood volume and cardiac output.

Indications (Therapeutic Uses)

This combination tablet is indicated for the management of essential hypertension when single-agent therapy provides insufficient blood pressure control:

  • Essential Hypertension (High Blood Pressure): Long-term management to reduce blood pressure in adult patients. Lowering blood pressure decreases the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.
  • Second-Line Combination Therapy: For patients whose blood pressure is not adequately controlled on telmisartan or hydrochlorothiazide monotherapy alone.
  • Initial Therapy Stabilization: May be used as initial therapy in patients who are likely to need multiple drugs to achieve their blood pressure goals.

Dosage & Administration

Website Note: Dosage must be individualized by a registered medical practitioner based on the patient’s therapeutic response and baseline renal and hepatic function.

  • Standard Dose: Typically one tablet (commonly formulated as 40mg/12.5mg or 80mg/12.5mg) taken orally once daily. Dosage adjustments should be made after at least 4 weeks of therapy if blood pressure remains uncontrolled.
  • Administration: Tablets must be swallowed whole with water. They can be taken with or without food, but should ideally be taken at the same time each day.
  • Storage Precaution: Telmisartan is highly hygroscopic (absorbs moisture from the air). Tablets should be kept in their original blister packaging until immediately before administration.

Contraindications & Precautions

  • Absolute Contraindications: Do not use in patients with known hypersensitivity to telmisartan, hydrochlorothiazide, or sulfonamide-derived drugs. Strictly contraindicated in patients with anuria (absence of urination), severe renal impairment (creatinine clearance less than 30 mL/min), severe hepatic impairment, or cholestasis. Co-administration with aliskiren is contraindicated in patients with diabetes.
  • Pregnancy Warning (Black Box Warning): Do not use during pregnancy. Drugs that act directly on the renin-angiotensin system can cause serious injury, developmental abnormalities, or death to the developing fetus when used during the second and third trimesters. If pregnancy is detected, discontinue this medication immediately.
  • Electrolyte Imbalances: Hydrochlorothiazide can cause hypokalemia (low potassium), hypomagnesemia, and hyponatremia, while telmisartan can potentially cause hyperkalemia. Regular monitoring of serum electrolytes is highly recommended.
  • Metabolic and Endocrine Effects: Thiazide diuretics may alter glucose tolerance and raise serum levels of cholesterol and triglycerides. They may also decrease urinary calcium excretion, potentially precipitating hyperuricemia or acute gout attacks in susceptible patients.
  • Systemic Lupus Erythematosus: Thiazide diuretics have been reported to exacerbate or activate systemic lupus erythematosus.

Potential Side Effects

  • Common / Mild: Dizziness, lightheadedness, fatigue, upper respiratory tract infections, diarrhea, nausea, and transient orthostatic hypotension (dizziness upon standing up).
  • Less Common / Severe: Severe hypotension/syncope (fainting), acute renal failure (especially in patients with bilateral renal artery stenosis), profound electrolyte imbalances, pancreatitis, worsening of gout, and hypersensitivity reactions (such as angioedema or photosensitivity).
