Mechanism of Action:
Dual-action glycemic control. Glimepiride stimulates insulin secretion from pancreatic beta cells, while Metformin decreases hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity.

Indications (Therapeutic Uses)

This combination tablet is indicated for the management of type 2 diabetes mellitus when dietary modification, physical exercise, and monotherapy do not result in adequate glycemic control:

  • Type 2 Diabetes Mellitus: Long-term management to improve glycemic control in adult patients.
  • Secondary Monotherapy Failure: For patients whose blood glucose levels are no longer adequately managed by metformin or a sulfonylurea alone.
  • Therapy Stabilization: To replace separate tablets of metformin and glimepiride given concurrently for ease of compliance.

Dosage & Administration

  • Standard Dose: Initially one tablet taken orally once daily. The dosage can be gradually titrated based on blood glucose monitoring.
  • Administration: Tablets must be swallowed whole with water. Do not crush, break, or chew sustained-release tablets, as doing so releases the entire dose at once, increasing side effect risks.
  • Timing: Must be taken with meals (typically with breakfast or the first main meal of the day) to minimize gastrointestinal side effects and reduce the risk of hypoglycemia.

Contraindications & Precautions

  • Absolute Contraindications: Do not use in patients with known hypersensitivity to glimepiride, metformin, or other sulfonylureas/biguanides. Strictly contraindicated in patients with type 1 diabetes, diabetic ketoacidosis, severe renal impairment (e.g., eGFR less than 30 mL/min), or acute/chronic metabolic acidosis.
  • Lactic Acidosis Warning: Metformin carries a rare but serious risk of lactic acidosis, a potentially fatal metabolic complication characterized by elevated blood lactate levels. The risk increases with conditions like renal insufficiency, sepsis, dehydration, and excessive alcohol intake.
  • Hypoglycemia Risk: Glimepiride can cause severe hypoglycemia (low blood sugar). Patients should be educated on recognizing symptoms (sweating, tremors, palpitations) and managing them with fast-acting carbohydrates.
  • Iodinated Contrast Media: Discontinue this medication temporarily at the time of, or prior to, iodinated contrast imaging procedures in patients with an eGFR between 30 and 60 mL/min, or a history of liver disease, alcoholism, or heart failure. Re-evaluate 48 hours post-procedure.

Potential Side Effects

  • Common / Mild: Hypoglycemia, gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal pain, flatulence, metallic taste), headache, and dizziness. Gastrointestinal symptoms are common during initial therapy and usually resolve spontaneously.
  • Less Common / Severe: Lactic acidosis, severe prolonged hypoglycemia, dermatological allergic reactions (pruritus, erythema, urticaria), hematological changes (leukopenia, thrombocytopenia), and impaired Vitamin B-12 absorption (with long-term metformin use).
