Mechanism of Action:
Suppresses gastric acid secretion by specifically inhibiting the H+/K+-ATPase enzyme system (the acid pump) at the secretory surface of the gastric parietal cell.

Indications (Therapeutic Uses)

Pantoprazole 40 mg is indicated for the targeted treatment and management of acid-mediated gastrointestinal conditions:

  • Gastroesophageal Reflux Disease (GERD): Short-term treatment (up to 8 weeks) in the healing and symptomatic relief of erosive esophagitis.
  • Maintenance of Healing of Erosive Esophagitis: Long-term management to reduce relapse rates of redness and inflammation in the esophagus.
  • Peptic Ulcer Disease: Treatment of gastric and duodenal ulcers to facilitate mucosal healing.
  • Zollinger-Ellison Syndrome: Long-term management of pathological hypersecretory conditions involving excessive gastric acid overproduction.
  • H. pylori Eradication: Combination therapy with appropriate antibiotics to eliminate H. pylori infections and reduce duodenal ulcer recurrence.

Dosage & Administration

  • Standard Dose: Typically 40 mg taken orally once daily. For hypersecretory conditions like Zollinger-Ellison syndrome, the initial dose may start at 40 mg twice daily and titrate higher if needed.
  • Administration: Tablets are enteric-coated and must be swallowed whole with water. Do not crush, split, or chew the tablets, as this destroys the protective coating required to bypass stomach acid.
  • Timing: Should be taken 30 to 60 minutes before a meal, preferably before breakfast.

Contraindications & Precautions

  • Absolute Contraindications: Do not use in patients with known hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation. Co-administration with rilpivirine-containing products is strictly contraindicated.
  • Clostridium difficile-Associated Diarrhea (CDAD): Decreased gastric acidity can increase the risk of severe gastrointestinal bacterial infections, including C. difficile.
  • Bone Fractures: Long-term, high-dose PPI therapy may increase the risk of osteoporosis-related fractures of the hip, wrist, or spine.
  • Hypomagnesemia: Severe low magnesium levels have been reported in patients treated with PPIs for at least 3 months, and in most cases, after a year of therapy.
  • Masking Malignancy: Symptomatic response to pantoprazole does not preclude the presence of gastric malignancy. Patients presenting with alarm symptoms (e.g., significant unintentional weight loss, recurrent vomiting) should be evaluated before starting therapy.

Potential Side Effects

  • Common / Mild: Headache, diarrhea, nausea, abdominal pain, flatulence, constipation, and dizziness.
  • Less Common / Severe: Acute interstitial nephritis (kidney reaction), cutaneous or systemic lupus erythematosus flare-ups, severe hypomagnesemia, Vitamin B-12 deficiency (with long-term daily use), and severe hypersensitivity reactions (anaphylaxis, Stevens-Johnson syndrome).
